'No-reflow' phenomenon.

چکیده
A 57-year-old man presented with new anginal symptoms nine years after three-vessel coronary artery bypass grafting. Cardiac catherization revealed severely and diffusely diseased saphenous vein graft to the obtuse marginal coronary artery. Percutaneous coronary intervention was complicated by the ‘no-reflow’ phenomenon. The patient suffered a periprocedural myocardial infarction (peak troponin 26.3 ng/mL) and died from low-output cardiogenic shock four days after the procedure. Gross autopsy findings demonstrated myonecrosis of the posterolateral wall (Fig. 1, black arrow). The infarction is in the distribution of the obtuse marginal coronary artery and extends further than the distribution of the obtuse marginal coronary artery secondary to low perfusion resulting from cardiogenic shock. Histopathologic examination revealed massive myonecrosis and abundant polymorphonuclear leukocytes consistent with recent myocardial infarction (Fig. 2). It is highly unusual and interesting that the hematoxylin and eosin stain demonstrated merely the inflammatory cells and necrosis, rather than debris resulting from procedure-related embolization. The ‘no-reflow’ phenomenon is defined as inadequate myocardial perfusion through a given segment of the coronary circulation without evidence of vessel obstruction. After placement of an embolic filter protection device, the graft was successfully recannalized with a bare metal stent. Immediately after the stent was placed, the ‘no-reflow’ phe-

No-Reflow Phenomenon in Patients with ST-Elevation Acute Myocardial Infarction, Treated with Primary Percutaneous Coronary Intervention: A Study of Predictive Factors
Introduction: No-reflow phenomenon in coronary vessels, manifested in some patients with reperfused acute myocardial infarction (MI), is associated with poor clinical and functional outcomes. Therefore, evaluation of predisposing risk factors can be helpful in risk assessment and identification of patients at higher risk. Herein, we aimed to study the predictive factors for the development of...
متن کاملTIMI Risk Score as A Predictor for No-Reflow Phenomenon in Patients Undergoing Primary Angioplasty for Acute STEMI
Background: Angiographic no-reflow phenomenon after primary Percutaneous Coronary Intervention (PCI) in patients with acute ST-elevation myocardial infarction (STEMI) may result in unfavorable outcome. Objectives: The aim of the study was to evaluate the TIMI risk score of STEMI as a predictor of no-reflow phenomenon after primary PCI. Patients and Methods: We analyzed 627 patients presenting w...
متن کاملRenal Dysfunction on Admission Predicts No-Reflow Phenomenon in Patients Undergoing Manual Thrombus Aspiration during Primary Percutaneous Coronary Intervention.
BACKGROUND No-reflow is a frequent complication during percutaneous coronary intervention (PCI) for acute ST segment elevation myocardial infarction (STEMI). Available data is limited regarding its impact on short-term outcomes in patients undergoing manual thrombus aspiration. Renal impairment is also associated with higher complication rates in STEMI. Herein, we aimed to evaluate the impact o...
متن کاملAngiographic no-reflow phenomenon as a predictor of adverse long-term outcome in patients treated with percutaneous transluminal coronary angioplasty for first acute myocardial infarction.
OBJECTIVES We sought to elucidate the long-term prognostic importance of angiographic no-reflow phenomenon after percutaneous transluminal coronary angioplasty (PTCA) for acute myocardial infarction (AMI). BACKGROUND Angiographic no-reflow phenomenon, a reduced coronary antegrade flow (Thrombolysis in Myocardial Infarction [TIMI] flow grade < or =2) without mechanical obstruction after recana...
متن کاملEvaluation of related factors, prediction and treatment drugs of no-reflow phenomenon in patients with acute ST-segment elevation myocardial infarction after direct PCI
This study determined the related factors of no-reflow phenomenon in patients with acute ST-segment elevation myocardial infarction (STEMI) after direct percutaneous coronary intervention (PCI), and evaluated related factor scores in predicting the occurrence of no-reflow phenomenon and drug treatments. A total of 203 patients with acute STEMI receiving PCI who were admitted to the Department o...
متن کاملPredictive factors for development of the no-reflow phenomenon in patients with reperfused anterior wall acute myocardial infarction.
OBJECTIVES We sought to elucidate the clinical factors related to the development of no-reflow phenomenon after successful coronary reperfusion in patients with an acute myocardial infarction (AMI). BACKGROUND Myocardial contrast echocardiography revealed that the no-reflow phenomenon is observed in some patients with a reperfused AMI, and those patients usually have poor functional and clini...
